Weekend Clinical Coordinator

Job Summary

NWVNA is looking for a Weekend Clinical Coordinator, reporting to Christine Ogden, Senior Director of Operations.

Responsibilities:

· Provides oversight of all patient care services and personnel.

· Supports the organization’s mission, vision, and values.

· Promotes superior customer service to internal and external clients, customers, patients, and staff.

· Ensures coordinated care of new and existing patients.

· Performs joint visits with clinicians, or delegates to an appropriate colleague as needed for a skill/complication.

· Schedules staff Saturday, Sunday, Monday, and as needed.

· Make welcome calls to new patients.

· Ensures fair and consistent workloads for all clinicians and home health aides relative to caseloads, productivity, and geography.

· Collaborates with QAPI leaders to assist in educating clinicians as well as hold them accountable to policies, procedures, and best practices.

· Collaborates with QAPI team to help ensure orders are approved by the responsible physician.

· Participates in the agency wide QAPI program.

· Swiftly communicates adverse events, complaints, or other untoward issues to the Senior Director of Operations.

· Operationalizes agency goals within the scope of the role.

· Participates and may lead task forces, as requested.

· Promotes compliance with all federal and state regulations.

· Manages special programs as needed.

· Identifies, and resolves, when possible, issues impacting clinical care and service.

· Continually assesses staffing needs for their respective team.

· Manages telephone triaging and customer services as needed.

· Acts as a helpful resource to clinicians.

· Provides direct patient care, if needed.

· Works in conjunction with the Clinical Manager

· Works as part of the intake process.

· Additional duties, as assigned.

Requirements/Qualifications:

· A licensed physical therapist, speech-language pathologist, occupational therapist, or registered nurse in Massachusetts. RN, or PT, preferred

· Master’s Degree in a clinical field or MBA, preferred.

· Proficient knowledge of home health care eligibility, services, and reimbursement guidelines.

· Three years’ experience in Home Health Care required.

· Valid driver’s license

· Exceptional communications skills: verbal, nonverbal, and written.

· Working knowledge of computer /office products and ability to quickly learn our home care software package and tools.

· Working knowledge of OASIS documentation.

· Demonstrated ability to effectively lead a team of diverse individuals.

Work Environment:

· Ability to work independently in the home care setting.

· Must demonstrate ability to work in a team-oriented environment.
