Natick Walpole VNA is seeking a Speech Therapist to work in the home care setting. Flexible schedule and competitive salary!
Duties will include:
- Assumes accountability for own professional practice in achieving patient outcomes
- Accurately, promptly and thoroughly documents patient care observations, interventions and evaluations, meeting agency parameters.
- Identifies primary diagnosis, reasons for home care, and problems based on assessment, which includes patient demonstration of functional status; then develops a plan of care that will best meet patient outcomes
- Responsible for initiating, evaluating and updating the patient plan of care to achieve desired outcomes
- Provides patient education; makes appropriate referrals based on identified patient needs that will maximize independence and patient/family participation
- Collaborates with the physician and other members of the health care team to ensure care is safe, effective and coordinated.
- Provides comprehensive home evaluations with recommendations
Requirements:
- Current licensure from the Board of Allied Health in the State of Massachusetts
- Current valid driver’s license
- Graduate of an accredited college of Speech Therapy
- Must have demonstrated competence in community health or clinical patient care setting, providing direct patient care with patients for a minimum of two years.

- Ability to work independently in the home care setting.
