Job Summary: To provide Physical Therapy services, under the supervision of a licensed Physical Therapist, to patients and families in their homes.
Responsibilities:
· Provide care and services in accordance with the plan of care.
· Assist community members and health field personnel to assess, plan for, and provide needed health and related services.
· Document care provided in an accurate and timely manner.
· Provides teaching to clients and families.
· Coordinate and collaborate with other members of the health care team, community agencies, and referral sources.
· Communicate the care plan with patients and families and insures understanding of the exercise program prescribed.
· Responsive to meeting or exceeding patient/client expectations and promotes a positive image of NWVNA.
· Adhere to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.
· Attends continuing education programs.
· Other duties assigned by the department manager.
Requirements/Qualifications:
· Associate’s degree and graduation from an accredited Physical Therapy program.
· One year of physical therapy experience is preferred.
· Possesses and maintains CPR Certification
· Possess valid driver’s license
