Job Summary: To provide Physical Therapy services, under the supervision of a licensed Physical Therapist, to patients and families in their homes.
Responsibilities:
- Provides care and services in accordance with the plan of care.
- Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
- Documents care provided in an accurate and timely manner.
- Provides teaching to clients and families.
- Coordinates and collaborates with other members of the health care team, community agencies and referral sources.
- Communicates the plan of care with patients and families and insures understanding of the exercise program prescribed.
- Is responsive to meeting or exceeding patient/client expectations and promotes a positive image of the NWVNA.
- Adheres to all agency policies and procedures.
- Attends continuing education programs.
Requirements/Qualifications:
- Associate’s degree and graduation from an accredited Physical Therapy program.
- One-year Physical Therapy experience preferred.
- Current valid driver’s license.
