Job Summary: To provide Physical Therapy services, under the supervision of a licensed Physical Therapist, to patients and families in their homes.

Responsibilities:

  • Provides care and services in accordance with the plan of care.
  • Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
  • Documents care provided in an accurate and timely manner.
  • Provides teaching to clients and families.
  • Coordinates and collaborates with other members of the health care team, community agencies and referral sources.
  • Communicates the plan of care with patients and families and insures understanding of the exercise program prescribed.
  • Is responsive to meeting or exceeding patient/client expectations and promotes a positive image of the NWVNA.
  • Adheres to all agency policies and procedures.
  • Attends continuing education programs.

 

Requirements/Qualifications:

  • Associate’s degree and graduation from an accredited Physical Therapy program.
  • One-year Physical Therapy experience preferred.
  • Current valid driver’s license.

  • Must demonstrate the ability to work in team-oriented environment.
  • Ability to work independently in the home care setting.
