LPN – HOME HEALTH NURSE
Natick Walpole VNA is one of the most trusted providers of home health care in the industry. We have been in business for over 100+ years caring for our patients in the MetroWest Communities we serve.
We are seeking a compassionate and dedicated LPN to join our team. We offer flexible scheduling, weekday positions with low frequency weekend rotation and a holiday requirement of 2 holidays per year. You will be an integral member of the clinical team and will collaborate with other multi-disciplinary members to meet the needs of the patient.
Position Details:
- Walpole and 40 surrounding towns
- Full-time and Part-time positions available
Responsibilities:
- Provide patient revisits in accordance with the plan of care established with the patient’s physician.
- Ability to function independently as a clinician in the community.
- Solid patient assessment skills
- Comfortable with using an EMR.
- Great communication skills
- Great critical thinking skills and ability to triage patients and problems.
Qualifications:
- Current LPN License from MA Board of Nursing.
- Two years’ experience in delivering nursing care.
- One year of home health care experience preferred.
