Medical Social Services

Our team of Social Workers assess your physical, emotional, environmental, and social needs and offer supportive resources in response to identified concerns and issues. Patient advocacy is also offered, as necessary.

Our Medical Social Services include:

• Helping patients adjust to an illness
• Facilitating a connection to community resources including:
  • Long-term care education and planning
  • Elder care services, including Meals on Wheels
  • Prescription, fuel and food assistance
  • Transportation
  • State and federal programs including Medicare, Medicaid, and Social Security
