Community Outreach Liaison

Job Summary

The Community Liaison interacts with hospitals, post-acute care providers and assisted living discharge planners and case managers at least weekly. Meets with clients and families at designated facilities and/or MD Offices and in the community, attends all business networking events, delivers presentations, participates in local provider meetings, and helps ensures agency admission goals are met.  The liaison promotes and encourages the referral of home healthcare services and the safe transition of patients to home.  Responsible for insuring complete, accurate patient information from referral sources to intake. Additional responsibilities include assistance in obtaining documentation for orders, Face to Face (FTF) documents and addendums.  The liaison works to develop and implement the agency’s marketing and business development plan under the supervision of the Director of Marketing and Business Development.

Responsibilities:

  • Generate quality referrals leading to admissions contributing to the monthly goals.
  • Establish ongoing productive relationships with referral sources.
  • Connecting with existing and potential referral sources working within an assigned territory to promote our continuum and increase referral base
  • Developing ongoing, productive relationships with referral sources with assigned territory by listening to needs, solving problems, and meeting expectations
  • Establish the potential patient’s eligibility for home care.
  • Educate referral sources, (skilled nursing facility staff, physicians, hospital staff, community agencies…) on our services, capabilities and territory and encourage partnership
  • Communicate with the patients and families of patients about services and resources available.
  • Works effectively with the Director of Marketing and Business Development to identify key accounts and interact with them (hospitals/SNFs/MD offices) on a regular schedule.
  • Assess patients upon admission to the hospital or medical facility for home health care service needs, as requested.
  • Coordinate with healthcare facility staff to ensure that each patient has a smooth and safe transition through the discharge process and back to the home environment.
  • Develop relationships in the community on behalf of the agency in accordance with the agency’s business development plan.
  • Represents interests of the agency at professional organization meetings, as needed.
  • Attend sales meetings, training sessions, and working with the sales team to develop strategies across all territories
  • Develop a sales plan for assigned territory with specific goals and objectives and actions to meet them.
  • Assists in obtaining documentation related to FTF’s, ADR’s, addendums, and physician orders.

Requirements/Qualifications:

  • Bachelor’s degree in marketing or relevant field
  • Proven success in an outside sales position.
  • Clinical experience in the home health or hospice setting. RN, LPN, and CNAs preferred.
  • Outside sales experience within the past 3 years.
  • Evidence of achieving referral goals within the market.
  • Excellent planning, organizational, and presentation skills.
  • Ability to travel to local facilities and physician offices regularly.
  • Knowledge of marketing and/or business development principles.
  • Exceptional written and verbal communication skills.
