Job Summary:

Natick Walpole VNA is seeking a full-time Clinical Manager for our growing team! This role is Monday through Friday, 8am to 4pm. We are an expanding agency with high expectations for growth.

Overview of Clinical Manager Role:

Incumbent will direct, coordinate, and evaluate the workflow and effectiveness of the care and service of the clinical team. The Clinical Manager constantly evaluates the team, validating the application of best practice care, determining need for staff education, identifying areas for greater efficiencies, and offering ideas to implement process improvement where and when necessary. The Clinical Manager is a member of the agency-wide management team.

Responsibilities

  • Provide oversight of all patient care services and personnel.
  • Support the organization’s mission, vision, and values.
  • Promote superior customer service to internal and external clients, customers, patients, and staff.
  • Accountable for the care and outcomes that the team is providing.
  • Ensure coordinated care of new and existing patients.
  • Meet at least every other week with clinicians to review plan of care, coordination of all disciplines involved and appropriate allocation of services.
  • Ensure appropriate orientation and mentoring of new clinicians.
  • Perform joint visits with clinicians, or delegates to an appropriate colleague, at least annually as a means of assessing clinician ability to manage patient care needs, establish a plan of care toward improved patient outcomes, and provide coaching and education to clinicians to promote accurate assessments and documentation.
  • Analyze patient data specific to clinicians and the team, noting trends to optimize patient safety, patient outcomes, and fiscal responsibility.
  • Ensure fair and consistent workloads of all clinicians and home health aides relative to caseloads, productivity, and geography.
  • Effectively Interviews and hires staff.
  • Collaborate with QAPI leaders to assist in educating clinicians as well as hold them accountable to policies, procedures, and best practices.
  • Collaborate with QAPI team to help ensure orders are approved by the responsible physician.
  • Participate in the agency wide QAPI program.
  • Facilitate effective, regular multi-disciplinary case management meetings.
  • Swiftly communicates adverse events, complaints, or other untoward issues to the senior operations manager.
  • Operationalize agency goals within the scope of the role.
  • Participate and may lead task forces, as requested.
  • Promote compliance with all federal and state regulations.
  • Manage special programs as needed.
  • Identify, and resolve, when possible, issues impacting clinical care and service.
  • Continually assesses staffing needs for their respective team.
  • Act as a helpful resource to clinicians.
  • Provide direct patient care, if needed.
  • Additional duties, as assigned.

Qualifications:

  • Registered nurse in Massachusetts.
  • Proficient knowledge of home health care eligibility, services, and reimbursement guidelines.
  • Three years’ experience in Home Health Care required.
  • Valid driver’s license
