Job Summary
Provide health care tasks, personal hygiene services to individuals in their home. Observe, record and report all changes to supervisor. All HHA’s must demonstrate competence in performing the necessary skills and only perform those skills on the plan of care.
Responsibilities:
· Assist with personal care, i.e. bathing, skin care, routine nail care, shampooing, toileting, feeding, dressing, positioning, transferring, shaving, mouth care.
· Perform certain activities under the supervision of an RN/PT/OT i.e. colostomy care, assist with oral medications, for example cuing, reinforce a dressing as directed, change nonsterile dressings, assist patient with ambulation device, specimen collection, measure intake and output, vital signs, oxygen safety.
· Carry out simple exercise program as taught by therapist
· Provide light cleaning of sick room, light meal prep
· Adhere to care plan; recognizes and reports changes in patient’s condition to nurse/therapist
· Maintain active driver’s license
· Ability to communicate with patients to determine their needs and explain treatment
· Participate in case conferencing when requested
· Attend mandatory staff meetings and education programs
· Promote a positive image of the agency.
· Maintain confidentiality on all agency matters.
· Adhere to all agency policies and procedures
Skills/Qualifications:
· Must have a current and valid HHA Certificate.
· Must be at least 18 years of age.
· Must have a phone or a way to communicate to the office daily.
· Must be able to arrive at cases at the required time.
· Must have a car.
· Must be able to follow the care plan written in the home.
· Physical exam must state that you are able to work with no restrictions and have been completed within one year of date of hire.
· Preferred high school diploma or equivalency
Requirements/Qualifications:
· Mental Demands: Job involves performing tasks under the direction and supervision of a registered nurse/therapist. Work requires adherence to precise procedures and standards involving a high degree of accuracy in observing, recording and reporting data.
· Physical Demands Appreciable physical effort or strain. Moderately heavy activity. May include lifting, constant stooping and walking.
· Working Environment: Continuous exposure to various disagreeable physical conditions.
Must demonstrate ability to work in team oriented environment.
· Contacts: Good communication skills required with patients, family and other employees.
